If you use the appointment and contact forms on our website, the following data may be processed:
- Identification and contact data: first name, last name, phone number, email address.
- Transaction data: the service you are interested in, your preferred appointment date, and the content of the message you shared in the form.
- Health data: health information you choose to share during the appointment consultation or in your message (this constitutes special-category personal data and is processed only with your explicit consent, for the purpose of planning the examination and treatment process).
3. Purposes of Processing Personal Data

- Planning and conducting examination and treatment processes,
- To fulfill legal obligations and to use such data as evidence in the event of potential disputes.
4. Legal Basis
Your personal data is processed based on the legal grounds specified in Articles 5 and 6 of the Personal Data Protection Law (KVKK): “the existence of explicit consent,” “being directly related to the conclusion or performance of a contract,” and “the legitimate interest of the data controller.” Your health data, however, is processed solely with your explicit consent.
5. Transfer of Personal Data
Your personal data may be shared with our hosting service providers—who operate our website and appointment system—and with legally authorized public institutions and organizations, to the extent necessary for the provision of our services and in accordance with the conditions set forth in the KVKK. Your data is not transferred abroad.
6. Retention Period
Your personal data is retained for as long as necessary to fulfill the purpose of processing and subject to the statute of limitations periods set forth in relevant legislation; at the end of this period, it is deleted, destroyed, or anonymized.
7. Your Rights
Pursuant to Article 11 of the KVKK, with regard to your personal data: you have the right to learn whether your data is being processed; if so, to request information regarding such processing; to learn the purpose of processing and whether it is being used in accordance with that purpose; to know the third parties to whom your data has been transferred within or outside the country; and to request the correction of any incomplete or incorrect data; request the erasure or destruction of your data under the conditions set forth in the KVKK, request that these actions be communicated to third parties to whom your data has been transferred, and object to decisions made solely through automated systems that result in adverse consequences for you.
8. Method of Submission
To exercise these rights, you may submit your requests, along with documents verifying your identity, via email to [email protected] or in writing to the address specified above. Your requests will be processed as soon as possible and no later than 30 days, depending on their nature.
